Assumes responsibility to assist the cardiovascular patient and the cardiologist in the inpatient setting. Collaborates with nursing colleagues in the care of the cardiovascular heart failure patients by providing support and education. Integrates outpatient care of heart failure patients and families with continued education & support through telephone follow-up. Favorably improves patient outcomes through providing expert consultation to colleagues on patient care problems and facilitates integration with physicians. Contributes to patient education and provides input into cost–effectiveness. In addition, provides input into development and implementation of new programs. Participates in the quality improvement activities, the TUL process and unit based governance activities to improve patient care and nursing practice. Registered nurse with a valid Indiana State RN license. 3 years critical care or related specialty area. CPR, ACLS, PALS. |
